Are you a 501(c)3 organization?
Answer: Yes, we are. Our tax id is 32-0299001.
How do I apply for assistance?
Answer: Our application can be found on our website under the application tab. If you do not have a printer at home, please send us an email with your mailing address.
I am currently battling cancer. Do I qualify for assistance from your organization?
Answer: Patient must reside in Illinois and be seeking treatment in Illinois (within a 30-mile radius of Oak Lawn, IL 60454). Assistance must be for CURRENT cancer treatment (active treatment is defined as in surgery and follow-up surgery, radiation, and/or chemotherapy).
What needs to be included with my application?
Answer: Application should be completed and have required signatures. A letter on facility letterhead affirming the patient is CURRENTLY receiving cancer treatment and a photocopy of your VALID Illinois Driver’s License or State I.D. MUST be included. If you are applying on behalf of a minor, we ask for a parent’s VALID Illinois Driver’s License or State I.D. Failure to include all of these items will cause your application to be rejected or delayed.
I am still battling cancer. I have had reoccurrence. Can I receive additional assistance?
We are a nonprofit organization that depends solely on donations. To help as many cancer patients as possible, we cannot always guarantee additional assistance can be provided. If it is a new calendar year, the patient must reapply by submitting a new application.
I live in another state, can you help?
Answer: No. We are a Southland/Chicago nonprofit and our requirement is that the patient live within 30 miles of Oak Lawn, IL (zip code 60454). We recommend that you reach out to your social worker for resources in your area.
Can I nominate an individual battling cancer?
Answer: No, we ask for personal patient information. Please tell the cancer warrior about our organization and refer them to our website.
